Clinical Care Coordinator
| Clinical Care Coordinator Department: Cardiovascular Service Line Category: Administrative Type: Full-Time Shift: Day Description: Major Position Responsibilities: · The RN Clinical Coordinator has regular internal contacts, which will involve hospital ancillary services (e.g. nursing, laboratory, radiology, purchasing, engineering, housekeeping, pharmacy) as well as contact with Physicians in the performance of duties and if applicable the RN Clinical Coordinator has regular external contacts which will involve (e.g. Home health, nursing homes, durable medical equipment, ambulance/transportation services, and product vendors). · Organizes and prioritizes clinical responsibilities, provides direct supervision to staff RN’s and collaborates with physicians on patient care issues and treatment modalities. Collaborates with other health care providers and acts as a patient advocate. · Provides direct patient care to a group of patients while utilizing the nursing process. · Manages clinical staff, functions, activities, and case management development by following clinical practice guidelines. Reviews payroll and clinical staffing to ensure staffing ratios are maintained according to clinic volume. Provides continuity of care for patients in collaboration with physicians and staffing with accountability for planning, implementation and evaluation of comprehensive cardiovascular care involving multiple disciplines and sites of care evaluating care given and intervening as necessary. · Implements and coordinates in collaboration with the Medical Director, Service Line Administrator, personnel and other resources to ensure quality services and outcomes; integrates the Quality Assurance Program with the hospital. Ensures database accuracy and utilizes database reports for Quality Assurance Program. Meets monthly with Service Line team to review, Outcome Reports and cardiovascular services data · Assists in fiscal responsibilities of clinic, such as, budget preparation and monitoring, reimbursement, purchasing, and maintaining efficiency in staffing and supplies. · Analyzes and evaluates information needs and trends regarding staffing, procedures, equipment, staff education and training toward the goals of quality; collaborates with Program Director and Medical Director to formulate corrective action plans. · Provides for mandatory education to clinic staff and client personnel, monitors safe practices, and develops corrective action plans. · Initiates and participates as a change agent to improve work environment, work process, or patient care including coaching, discipline, and performance appraisals. · Obtains by self-initiative, advanced knowledge and credentials, and serves as a role model/educator; implements standards of care and adheres to policy and procedures and all regulatory requirements (e.g. JCAHO, Medicare, and FDA). · May assist or participate in providing community education. · Coordinates care paths for patients between primary care providers and cardiovascular specialist to assure clear communication and optimum service delivery · Provides exceptional customer relations · Ability to organize and oversee patient care throughout the continuum assuring that patients and families have clear expectations and understanding of services · Serves as Chest Pain Center (CPC) Coordinator and supports initiatives to maintain CPC certification standards Safety and Infection Control Responsibilities Responsible to maintain a safe and clean work environment, including unit based specific safety and infection control requirements. Qualifications: ESSENTIAL COMPETENCIES, KNOWLEDGE, & EXPERIENCE · Minimum five (5) years’ clinical experience; ambulatory experience preferred. · Minimum of two (2) years of prior supervisory or health care management experience required. · Demonstrates computer skills with database and appropriate software required. · Demonstrates advanced knowledge related to cardiovascular care by the ability to review cases for quality. · Ability to communicate in a professional manner required. · Excellent interpersonal communication skills required. Education: · Bachelor’s degree required. · Advanced degree and/or formal management education preferred. Licenses: · Current CA Registered Nurse licensure with state in good standing, required.
